Insurance coverage

Use the following to obtain liability insurance coverage for shows and trials held in the name of the ABTC. ABTC insurance is only for liability coverage (no worker's comp) and covers both the hosting group's ABTC members and any other non-ABTC members assisting them. ABTC show events will be covered by the Club's liability insurance policy ONLY if the following procedures are followed:

Requests must be sent to Lee Slorah, at LEE_SLORAH@peoplePC.com, at least 10 days or more in advance of the date of your event(s). If there is more than one facilities owner/lessor (like at the national or regional specialties), request all certificates at the same time, one for each owner/lessor.

Complete and submit (via e-mail when you click on the "submit" button below) the on-line form below, or print it, fill it in, and mail to Lee Slorah.

1. The deadline date needed for this certificate to arrive

2. Lessor / facility owners name

3. Name of the Event(s).
This must read "ABTC ......." like "ABTC Obedience Trial" or "ABTC Conformation & Agility Trials." Your clubs' name must not appear in the event's name, nor may it appear in any premium list or advertising.
4. Date(s) of Event(s)
5. The Street Address, City and State where the event will be held (NO PO Boxes)

6. The Name(s) & Address(es) where the Certificates will be sent. Certificates are normally sent to the facility / owner of the property. Provide their name & addresses, including zip code, for copies of the certificate. Include fax number(s) when available.

7. The names of "Additional Insured(s)" only when specifically required by the owner. Hosting clubs may not be additional insureds.

9. Name and address of person requesting this certificate.
